HealthChoice is a preferred provider (PPO) health plan. That means you can receive the highest level of benefits when you use any of the physicians and other health care professionals in the plan’s network. Benefits are also available for care delivered by health providers that are not part of the network, however benefits are often lower and your out-of-pocket costs will be greater. |
HealthChoice Standard pays 80% of most covered expenses after your deductible is met. You are responsible for paying the remaining 20%, but only up to a maximum of $1,000 per calendar year. |
HealthChoice Basic pays 60% of most covered expenses after your deductible is met. You are responsible for paying the remaining 40%, but only up to a maximum of $1,000 per calendar year. |
